  • Short breaks
  • Long-term support

Who we are

The Gwynedd and Anglesey Shared Lives Scheme provides person-centred support for adults who need help to live independently, stay safe and take part in their community.

Shared Lives offers an alternative to more traditional forms of care and support. People are supported by approved Shared Lives enablers in a welcoming home environment. Support may take place in the enabler's home, during the day, or through short-term arrangements, depending on the individual's needs.

Who the scheme supports

The scheme supports adults across Gwynedd and Anglesey, including:

  • Older people
  • People with learning disabilities
  • People with physical disabilities
  • People with mental health needs
  • People who need additional support during a period of change in their lives

Shared Lives support is based on each person's individual needs, strengths and goals. We help people develop skills, maintain relationships and take part in their community.

Shared Lives enablers are assessed, approved, trained and supported by the scheme. They provide practical support, encouragement and opportunities for people to build confidence and independence.

Our aim is to provide safe, positive and supportive arrangements where people feel valued, respected and included.

What we do

We match people who need support with approved Shared Lives enablers who can provide the right support in a safe and welcoming environment.

Every Shared Lives arrangement is planned around the individual. We take time to understand what support a person needs, what they would like to achieve and the type of environment that will suit them best. We also consider the skills, experience and interests of the Shared Lives enabler to help ensure a suitable match.

The scheme can support people through long-term arrangements, short breaks, day support and Stepping Forward Support. Arrangements are reviewed regularly to ensure they continue to meet the person's needs and agreed outcomes.

Through the scheme, people may be supported to:

  • Build confidence and independence
  • Develop daily living skills
  • Take part in activities they enjoy
  • Maintain routines and structure
  • Make and maintain relationships
  • Access local opportunities
  • Feel included in family and community life
  • Work towards personal goals
  • Receive support during a period of change or recovery
  • Enjoy a short break in a safe and supportive setting

We also recruit, assess, train and support Shared Lives enablers through regular contact, reviews and ongoing guidance.

Shared Lives is about building positive relationships and helping people feel connected, supported and valued.

Types of support

Shared Lives support can be arranged in different ways depending on the needs and circumstances of the person receiving support.

Long-term support

A person lives with a Shared Lives enabler as part of their household and receives regular support in a family-based environment.

Short breaks or respite

A person stays with a Shared Lives enabler for a short period. This provides a positive break for the individual and time for families or unpaid carers to rest.

Day support

A person spends time with a Shared Lives enabler during the day, taking part in activities, developing skills and accessing opportunities that support their wellbeing and independence.

Stepping Forward Support

This provides short-term support for people who need help to build confidence, develop daily living skills or move towards greater independence.

Stepping Forward Support may help someone after a change in circumstances, a period of illness or a hospital stay.

Every arrangement is carefully matched and planned to ensure it meets the needs of both the individual and the Shared Lives enabler.

Offering support

As a Shared Lives enabler, you can support adults to live independently and take part in their community.

You may help someone with daily routines, practical skills, day-to-day activities, short breaks or longer-term support arrangements. You could also support someone through Stepping Forward Support as they work towards greater independence.

The Gwynedd and Anglesey Shared Lives Scheme provides:

  • Training and guidance
  • A full assessment and matching process
  • Ongoing support from the Shared Lives team
  • Regular reviews and contact
  • Payment for the support you provide
